AC Cugini Forms

• AC Cugini Code of Conduct: To be completed by all players and one parent on AC Cugini travel teams or SFL teams.

• Travel Team Tryout /Medical Release/Liability Waiver Form: To be completed and signed by parent of any non-Cugini registered player and submitted to the AC Cugini head coach before  a player's first tryout or guest player's  first time participating with an AC Cugini team in any practice,  scrimmage, tournament, or league game. The form may also be emailed in advance to the AC Cugini head coach.
